Medikamentbruk og fall. Eit kvalitetsforbetringsprosjekt med klinisk audit som metode for å auke fokuset på medikamentbruk og fall blant sjukeheimsbebuarar

Abstract

Kvalitet og kvalitetsutvikling i helsetenesta er satsingsområde både internasjonalt, nasjonalt og kommunalt. Likeins er det stor satsing på kunnskapsbasert praksis, der målet er at diagnostisering og behandling skal vere kunnskapsbasert. Til tross for dette, ser ein ofte at det er eit gap mellom det vi gjer og det vi veit vi burde gjere. Føremålet med dette prosjektet var å bidra til auka merksemd på medikamentbruk og fall blant sjukeheimsbebuarar. Først ved å sjå om praksis på området var i tråd med det litteraturen beskriv som beste praksis, og vidare ved å kartlegge korleis fall i sjukeheimar vart dokumentert. Gjennom klinisk audit har 676 personar ved tre sjukeheimar blitt kartlagde i eitt år. Medikamentbruk ved fall, om retningslinje for førebygging av fall blant eldre menneske vart følgd og korleis fall i sjukeheimar var dokumentert har blitt kartlagt. Dei tre første trinna i auditsyklusen blei utført. Resultata frå auditen synte at ein stor del av sjukeheimsbebuarane som falt, nytta psykotropiske medikament på falldato. Få av bebuarane hadde fått ei vurdering av medikamentordinasjonen sin, og svært få hadde fått medikamentvurdering av ein spesialist. Mange av falla var berre dokumenterte i fritekst i journal, og dermed har ikkje rutinane om dokumentasjon av fall i skaderegistreringsskjema blitt følgt. På bakgrunn av dette kan ein konkludere med at praksis ved sjukeheimane ikkje er i tråd med anbefalingar i litteraturen når det gjeld medikamentbruk og fall, og at dokumentasjonssystemet deira heller ikkje er tilfredsstillande. Gjennom tilbakemelding til praksis om korleis praksis er, meir fokus på medikamentbruk og fall og opplæring i dokumentasjonsrutinane, vil praksis kunne forbetrast slik at den vert i tråd med det forsking tilrår.

Quality and quality improvement within health care are areas of international, national and local priority and concentration. Great emphasis is placed on evidence based practice, and the aim is to base diagnostic analysis and treatment on knowledge. Despite this, a gap between what is done and what should be done is observed. The aim of this project is to enhance the attention given to drug utilization and accidental falls among nursing home residents. Firstly, to see if everyday practice follows what the literature describes as best practice, and furthermore, to map out how accidental falls in nursing home residents are being documented. Through a clinical audit 676 persons within three nursing home residents have been registered for a year. Drug utilization at the time of falling, whether the clinical practice guidelines for the assessment and prevention of falls in older people were followed, and how falls have been documented in nursing home residents, were subject to registration. The three first steps in the audit process were carried out. The results from the audit showed that a large amount of the nursing home residents that fell used psychotropic medication at the time of falling. Few residents received evaluation of their medical prescription, and even fewer had their medical prescription evaluated by a specialist. Numerous accidental falls were only reported in the free text search in the journal, and the guidelines have therefore not been followed. In the light of this one can conclude that everyday practice in the nursing home residents do not follow what the literature describes as best practice when it comes to drug utilization and accidental falls, and their systems of documentation are not satisfactory. Through feedback to the practice field, more focus on drug utilization and accidental falls, and training in routines of documentation, everyday practice will be enhanced and developed to be in line with recommendations from the field of research.
